Sometimes unforeseen life circumstances can bring you to a place that you never expected. For individuals who have resided in skilled nursing facilities for over 60 days, this is a very evident reality.

Did you know that there are state programs that can help individuals in skilled nursing facilities transition back home? L.I.F.E. Inc. is proud to be a Lead Organization with the State of California's Department of Health Care Services for the California Community Transition (CCT) Project. The CCT Project is a Medi-Cal funded program designed to help individuals who have resided in skilled nursing facilities for over 60 days move back home and into the community.

For many individuals who find themselves in this situation, the thought of beginning the process to move back home can often seem overwhelming. What if I don't have a home to return to? What if I don't have any loved ones to help care for me when I leave the facility? What if I don't have the funds to help find housing or modify my home to ensure that I will be safe? What if I don't have anyone to help me fill out all of the paperwork?

This is where L.I.F.E. Inc. can step in. At L.I.F.E. Inc. we feel strongly that no one should feel trapped due to a lack of resources. Our Transition Coordinator, along with our team of nurses, will help to not only familiarize you and your loved ones with the CCT Project, but also check your eligibility, enroll you in the program, and assist and follow you in your transition for 365 days.

Elderhelp program

We are proud to be a provider with the Ventura County Area Agency on Aging (VCAAA).

The ElderHelp Program is an in-home service that provides assistance with personal care, homemaking, chores, food boxes, emergency material aids, minor home repairs, home modifications, and/or security devices. This program is available for frail senior residents of Ventura County.

multipurpose senior services program (mssp)

The VCAAA’s Multipurpose Senior Services Program pairs Ventura County residents over 65 with Care Managers to coordinate client needs. Residents must be enrolled in Medi-Cal.

Care Managers will help coordinate services including:

  • Community-Based Adult Services (formerly Adult Day Health Care)

  • Medical Equipment (Grab Bars, Bath Chairs, etc.)

  • Non-medical Equipment (Medical Alert Systems, Ramps, Heaters, Fans, etc.)

  • Supplemental Personal Care, Homemaker Chore Services, Caregiver Relief

  • Transportation

  • Minor Home Repairs

  • Counseling for Mental and/or Medical Issues
